Important Information:
All 6th grade students are required to have a physical examination completed for the 2022-2023 school year if you have not done so yet, please arrange to have done and paperwork turned into Mrs. Buchanan.
All 7th grade students are required by the Pennsylvania Dept. of Health to receive the MCV (Meningococcal) and Tdap Vaccinations and a Dental Exam, if you have not yet done so, please have the documentation turned into Mrs. Buchanan by November 14th, 2022.
Each school year a new permission form needs to be filled out to give your child over the counter medications in school (i.e Tylenol, Ibuprofen, Tums) this form can be located on the left of the screen under Over the Counter Medication Permission Form 2022-2023. Please fill out and return to school nurse.
**Please remember to update the emergency card in Skyward.**
If your child requires daily medication to be given during the school day, please fill out the form for Authorization of Medications and send to the school along with the medication prescribed.
